# Patient Forms

New patient, returning patient, child and records release paperwork can be completed before your visit. Ask the office to send the current forms when you book, so the appointment time goes on your eyes.

## Which paperwork applies

- New adult patient: full history, current medications, insurance details and consent.
- New child patient: as above, plus developmental history and school or teacher observations.
- Returning patient with changes: a shorter update covering address, insurance and any change in health or medication.
- Records release: to have your notes and imaging sent from a previous practice, or sent onward from here.

## How to get the forms

Ask when you book and the office will send the current versions to you. The forms are held by the practice rather than published here, which keeps a single current copy in circulation and avoids the situation the previous website had, where paperwork was being served from a staging hostname over an insecure connection.

## Records from a previous practice

A records release lets your previous optometrist or ophthalmologist send your history, prescriptions and imaging here. It is genuinely worth doing rather than starting from nothing, because much of eye care depends on comparing this year against previous years.

Previous corneal imaging matters particularly if you have keratoconus, and previous prescriptions matter particularly for a child whose myopia is progressing.

## Privacy

Protected health information created and held in the course of care is governed by the practice's Notice of Privacy Practices, which is available at the office. That is a separate document from this website's privacy policy and it takes precedence in relation to health information.
